Job summary

Focused Post Acute Care Partners FPACP, operating Focused Care at Clarksville, TX, seeks a Speech Language Pathologist to evaluate patients and develop language therapy programs in collaboration with physicians.

You will instruct, motivate and assist patients to reach therapy goals, document progress and communicate with the Rehab Director and care team. A Master’s in Speech-Language Pathology and valid state licensure are required, with strong communication and patient-centered care essential.

Qualifications

• Master's degree in Speech-Language Pathology and active state licensure are required.
• Ability to perform diagnostic speech and language assessments and document findings.
• Strong communication and collaboration with physicians and care teams.

Responsibilities

• Establish and evaluate treatment programs, goals, and discharge plans.
• Implement and adjust speech therapy plans in coordination with the physician.
• Administer diagnostic tools and document patient progress.
• Coordinate therapy with other clinicians and maintain accurate records.
• Adhere to Medicare guidelines and obtain required authorizations.
