Job summary

The Grand Rehabilitation and Nursing at Queens is seeking a Speech Language Pathologist. The SLP will provide and direct therapy services, conduct assessments, develop and implement plans, and perform duties under physician orders.

• Evaluates patients and develops data for treatment planning.
• Interprets evaluation findings to patients, family, and team members.
• Establishes patient schedules with other team members.
• Develops treatment plans with goals and methods.
• Coordinates treatment plans with patient and family.
• Monitors response to intervention and adjusts as needed.
• Recommends equipment to improve communication and trains users.
• Participates in discharge planning.
• Develops home/community programs to maintain progress.
• Documents outcomes and terminates services.
• Supervises students and volunteers per regulations.
• Maintains continuing education and professional development.
