Job summary

The Grand Rehabilitation and Nursing at Queens is seeking a Speech Language Pathologist to provide and direct Speech Therapy services to patients, including assessment and treatment, program development, and related in-services. Duties are under physician orders and include screenings, evaluations, treatment planning, and discharge planning.

The role requires a Master’s in Speech-Language Pathology and current state licensure or licensure eligibility, with strong communication and teamwork

Responsibilities

• Provides Speech Therapy services to patients and initiates referrals when appropriate.
• Conducts screenings to determine need for intervention/treatment.
• Evaluates patients to obtain data necessary for treatment planning and implementation.
• Interprets and communicates evaluation findings to patients, family, and other team members.
• Develops treatment plans by identifying a problem list and goals.
• Coordinates treatment plan with patient, family, and other team members.
• Monitors patient’s response to intervention and modifies treatment as needed.
• Recommends and trains on augmentative communication devices.
• Participates in discharge planning and home/community programming.
• Documents results of assessments and treatment.
• Supervises students and volunteers as assigned.
• Maintains confidentiality and adheres to infection control standards
