Qualifications

• Must hold Speech-Language Pathology licensure in the state where applying.
• 2+ years of clinical experience preferred.

Responsibilities

• Direct all aspects of quality patient care, including evaluation and treatment.
• Provide professional treatment for speech, language, swallowing, and cognitive disorders.
• Plan activities and deliver therapeutic services for speech and language disorders.
• Advocate for patients and participate in QA and customer service activities.
• Perform patient evaluations and develop treatment plans with short- and long-term goals.
• Progress patients within the established care plan and document progress.
• Develop and obtain approval for services from referring physician.
• Participate in patient care planning and utilization review meetings.
• Demonstrate skills to provide SNF-appropriate patient care.
• Administer standardized assessments and monitor patient progress.
• Document evaluations, treatment progress notes, and discharge summaries.
