Job summary

Pro Rehab is seeking a Speech Language Pathologist to float between Truman Senior Living and Park View Care Center in Minnesota. The position is on-call with a minimum of 6 hours per month, offering $45–$55 per hour depending on experience. Mileage reimbursement is provided at $0.76 per mile.

The SLP must hold CCC-SLP certification and an MN license. Responsibilities include assessing communication disorders, developing treatment plans, providing therapy, educating patients and families, and

Qualifications

• Must be a graduate from an Accredited Speech Therapy Program and have a bachelor's degree.
• Must possess a current accredited MN Speech Therapist License.
• Homecare Homebase EMR experience helpful but will train
• Experience with OASIS preferred or willing to be trained
• Must possess strong knowledge regarding state, federal, and local regulations as they pertain.
• Develop care plans and discharge planning strategies for optimal patient recovery.
• Collaborate with healthcare professionals to ensure comprehensive and personalized treatment plans.
• Demonstrate excellent communication skills to educate patients and their families on therapy techniques.
• Must be capable of performing the essential functions of the job, with or without reasonable accommodations.
• Must be able to communicate in English, both verbally and in writing and possess sufficient communication skills to perform the tasks required.

Responsibilities

• Assess & Diagnose Communication Disorders: Evaluate patient needs and deficits such as speech, language, voice, fluency, and swallowing difficulties.
• Develop & Implement Treatment Plans: Create personalized therapy programs to improve functional deficits.
• Provide Speech & Language Therapy: Conduct exercises and interventions to improve communication and speech abilities.
• Educate & Support Patients & Families: Teach strategies for effective communication and techniques to improve daily life.
• Monitor Progress & Adjust Treatment: Track patient progress and modify therapy plans as needed.
• Collaborate with Healthcare Teams: Work with doctors, nurses, and therapists to ensure comprehensive support.
