Job summary

Community First Solutions is seeking a Licensed Speech-Language Pathologist (SLP) to join our Therapy Department at Jamestowne, a short-term rehab facility with outpatient services. You’ll provide evaluations, treatment planning, reassessments, and discharge planning, collaborating with residents, families, and healthcare professionals to enhance independence, safety, and quality of life.

You must hold an Ohio SLP license with a Master’s degree and have at least two years of SLP experience,

• Master’s degree in Speech-Language Pathology from an accredited program.

Responsibilities

• Administers speech-language pathology evaluations, treatments, and discharge planning.
• Works with residents, families, and health professionals to optimize communication and swallowing outcomes.
• Develops individualized treatment plans and documents progress and outcomes.
• Maintains infection control and safety standards in all therapy activities.
• Collaborates with an interdisciplinary team to ensure high-quality resident care.

We are seeking a Licensed Speech-Language Pathologist (SLP) to join our Therapy Department at Jamestowne, a short-term rehab facility with an additional focus on outpatient services, with Community First Solutions. In this role, you’ll provide comprehensive speech-language pathology services including evaluations, treatment planning, ongoing reassessments, and discharge planning while collaborating with residents, families, and healthcare professionals. You’ll diagnose communication and swallowing disorders, develop individualized treatment programs, and support residents in regaining or strengthening speech, cognition, and functional communication skills. Responsibilities

• Administers physical therapy in compliance with Federal, State, local, and Community First Solution’s standards, policies, and guidelines ensuring quality care and service to Community First Solution’s residents.
• Interacts with health professionals, residents, families, and support groups to ensure the maximum rehabilitation, while serving as a client advocate at all times.
• Evaluates residents by administering muscle, neurological, gait, and functional tests.
• Establishes treatment goals based on the evaluation of the resident in conjunction with the resident’s needs.
• Develops treatment programs and provides ongoing assessments and modification as changes in resident’s condition occur.
• Prepares, transmits, and maintains timely and accurate records to include the initial evaluation, progress notes, discharge summary, and attendance records.
• Complies with OSHA standards and ensures the safety of the resident in their care at all times.
• Adheres to infection control policies, professional therapeutic services policies and all Community First Solution’s policies and procedures.
• Adheres to all policies and procedures of the organization and all federal and state laws regarding the privacy of protected health information.
• Adheres to all policies and procedures of the organization regarding the use of technology and technology-based information.
• Responsible for welcoming and on-boarding new employees.

Qualifications

• Education: Bachelor’s degree from an accredited college in Physical Therapy (Master’s degree preferred).
• Licenses or other required certifications: Current Ohio Physical Therapy License.
• Experience: At least two years physical therapy experience in long-term health care setting.
