Job summary

Community First Solutions is seeking a Licensed Physical Therapist to join the Jamestowne Therapy Department, a short-term rehab facility with outpatient services. The PT will conduct evaluations, plan treatments, reassess progress, and discharge residents while coordinating with families and healthcare professionals.

Strong collaboration and documentation skills are essential to enhance mobility and independence.

Qualifications

• Bachelor's degree in Physical Therapy required; Master’s degree preferred.
• Current Ohio Physical Therapy license required.
• At least two years of physical therapy experience in long-term health care setting.

Responsibilities

• Administers physical therapy in accordance with Federal, State, local, and Community First Solutions standards.
• Evaluates residents using muscle, neurological, gait, and functional tests.
• Develops and reassesses treatment plans; sets goals with residents.
• Maintains accurate records (initial evaluation, progress notes, discharge summary).
• Ensures infection control and resident safety at all times.
• Collaborates with interdisciplinary team and communicates with families.

Responsibilities

• Administers physical therapy in compliance with Federal, State, local, and Community First Solution’s standards, policies, and guidelines ensuring quality care and service to Community First Solution’s residents.
• Interacts with health professionals, residents, families, and support groups to ensure the maximum rehabilitation, while serving as a client advocate at all times.
• Evaluates residents by administering muscle, neurological, gait, and functional tests.
• Establishes treatment goals based on the evaluation of the resident in conjunction with the resident’s needs.
• Develops treatment programs and provides ongoing assessments and modification as changes in resident’s condition occur.
• Prepares, transmits, and maintains timely and accurate records to include the initial evaluation, progress notes, discharge summary, and attendance records.
• Complies with OSHA standards and ensures the safety of the resident in their care at all times.
• Adheres to infection control policies, professional therapeutic services policies and all Community First Solution’s policies and procedures.
• Adhere to all policies and procedures of the organization and all federal and state laws regarding the privacy of protected health information.
• Adhere to all policies and procedures of the organization regarding the use of technology and technology-based information.
• Responsible for welcoming and on-boarding new employees.

Qualifications

Education: Bachelor's degree from an accredited college in Physical Therapy (Master’s degree preferred).

Licenses or other required certifications: Current Ohio Physical Therapy License.

Experience: At least two years physical therapy experience in long-term health care setting.
