Qualifications

• Current Ohio Physical Therapy License.
• At least two years physical therapy experience in long-term health care setting.

Responsibilities

• Administers physical therapy in compliance with standards and guidelines.
• Interacts with health professionals, residents, families, and support groups.
• Evaluates residents by administering tests.
• Establishes treatment goals based on evaluation.
• Develops treatment programs and provides ongoing assessments.
• Maintains timely and accurate records (evaluation, progress notes, discharge summary).
• Adheres to infection control and safety policies.
• On-boards new employees.
