Job summary

Evolve Therapy Services, LLC seeks a Speech Language Pathologist for an in-house Skilled Nursing Facility in Meyersdale, PA. This PRN position focuses on delivering high-quality speech therapy and coordinating care with a multidisciplinary team.

Responsibilities include evaluation, treatment planning, and targeted goal development, with CMS-compliant documentation in NetHealth EMR. Active SLP licensure and CCC are required to join a supportive clinical environment.

Qualifications

• Master's degree in Speech Language Pathology required.
• CCC (Certification of Clinical Competence) expected or required.
• Active SLP license in state(s) of practice is required.
• Ongoing continuing education to maintain licensure.

Responsibilities

• Provide quality speech therapy services and initiate referrals in accordance with standards.
• Evaluate and obtain data for treatment planning and implementation.
• Develop treatment plans with targets and goals.
• Instruct patient, family, and staff to ensure carryover of goals.
• Complete compliant documentation for CMS expectations in NetHealth EMR.
• Oversee protocols for FEES/MBS with site coordination.
• Screen residents to identify needs and maintain clinical excellence.

Job Position: Speech Language Pathologist for an In-house Skilled Nursing Facility

Job Type: PRN

Job Purpose: To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions could include:

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.
• To evaluate and obtain data necessary for treatment planning and implementation
• To develop a treatment plan that includes a target list, short-term, and long-term goals
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities
• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems
• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs
• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications:

• Master's degree in Speech Language Pathology
• Certificate of Clinical Competency
• MUST have active SLP license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification
