Matter: Evaluate patients for speech and language disorders, initiate
assessments upon referral, and develop individualized plans of care in
collaboration with the physician and multidisciplinary care team.
Provide rehabilitative speech-language therapy services, carry out
appropriate interventions, and educate patients, families, and
caregivers on treatment procedures and supportive activities. Monitor,
document, and report patient progress, responses to therapy, and changes
in condition using the agency's Problem Oriented Record (POR) and
required clinical documentation systems. Coordinate care with healthcare
team members, consult with supervisors, and participate in case
conferences, in-services, and ongoing professional development. Support
patients and families in adapting to physical, mental, and social
limitations by providing education, encouragement, and resources to
promote independence and continuity of care. Maintain compliance with
agency policies, patient rights, confidentiality, infection control,
safety standards, and emergency preparedness while managing workload,
