Job summary

Legacy Lifecare VNA/Hospice, based in Massachusetts, seeks a Speech-Language Pathologist to evaluate and treat patients, develop care plans, and collaborate with a multidisciplinary team. This PRN role offers flexible daytime hours and compensation per assessment/session.

Responsibilities include monitoring progress, documenting outcomes, coordinating with healthcare staff, and educating patients and families to promote independence and continuity of care.

Qualifications

  • Active MA Speech-Language Pathologist license.
  • 1–3 years of experience in geriatrics, long-term care, or short-term rehab.
  • Ability to communicate fluently in English (written and spoken).

Responsibilities

  • Evaluate patients for speech and language disorders, initiate assessments upon referral, and develop individualized plans of care in collaboration with the physician and multidisciplinary care team.
  • Provide rehabilitative speech-language therapy services, carry out appropriate interventions, and educate patients, families, and caregivers on treatment procedures and supportive activities.
  • Monitor, document, and report patient progress, responses to therapy, and changes in condition using the agency's POR and required clinical documentation systems.
  • Coordinate care with healthcare team members, consult with supervisors, and participate in case conferences, in-services, and ongoing professional development.
  • Support patients and families in adapting to physical, mental, and social limitations by providing education, encouragement, and resources to promote independence and continuity of care.
  • Maintain compliance with agency policies, patient rights, confidentiality, infection control, safety standards, and emergency preparedness while managing workload, scheduling, and priorities effectively.

$110/per assessment | $75/routine therapy sessions | $45/hr for administrative work

Qualifications For Success
