Job summary

THE LAKES COMMUNITY HEALTH CENTER INC is seeking a Speech Pathologist who will prepare evaluations, diagnose communication issues, and create treatment plans. The role requires a graduate from an accredited program and a valid license to practice.

Job description

Salary begins at $68,511 + per year depending on experience based on a 1.0 FTE. Position could be 24 to 40 hours per week.

Essential Job Functions
  • Prepare for, administer, score, and interpret initial evaluations and re-evaluations including writing a report.
  • Communicate with patients to evaluate their levels of speech or language difficulty.
  • Determine the extent of communication problems by having a patient complete basic reading and vocalizing tasks or by giving standardized tests.
  • Diagnose & identify treatment options.
  • Create and carry out an individualized treatment plan.
  • Teach patients how to make sounds and improve their voices.
  • Teach alternative communication methods, such as sign language, to patients with little or no speech capability.
  • Work with patients to improve their ability to read and write correctly.
  • Work with patients to develop and strengthen the muscles used to swallow.
  • Counsel patients and families on how to cope with communication disorders.
  • Review relevant literature, synthesize evidence‑based materials, and translate complex behavioral health concepts.
  • Conduct case management and other administration tasks as needed.
  • Provide referral services to outside therapists, clinics, and treatment facilities.
  • Refer patients to specialists as needed.
  • Collaborate with NorthLakes providers as well as external resources and providers in the best interest of the patients’ continuation of care.
  • Work collaboratively with NorthLakes support staff in the interest of care integration initiatives.
  • Plan and administer therapeutic treatment by telephonic/telehealth methodologies as indicated by patient needs and state guidelines.
  • Appropriate use of PPE/infection control protocols when providing treatment to patients.
  • Other duties may be assigned.
Qualifications and Education Requirements
  • Graduate of Accredited Speech Pathology Program.
  • Current, valid, and unrestricted license to practice the profession.
Preferred Skills
  • Experience with Electronic Health Record.
  • Microsoft Office.
  • Google.
