You will educate patients and their family members and ensures the safety of the patient.**MAJOR AREAS OF RESPONSIBILITY:** Plan of Care: Develop an individualized ST plan of care and makes adjustments as needs change. Guide the patient in his/her use of therapeutic, creative and self-care activities for the purpose of improving functioning. Observe, record, and report patient reactions, changes, and progress to attending physician and any appropriate agency staff.Patient Outcomes: Use activities and home exercise programs to improve swallowing, breathing, cognition, speech and communication, and/or any other speech therapy related function.Minimize Patient Risk: Help decrease re-hospitalizations by teaching the patient on preventative measures and making good decision.Family Educator/Advocate: Counsel patient and family on the disease/injury and how to manage in all phases of speech therapy. Counsel and instruct patient and family on the disease/injury/disability and how to manage in all phases of speech therapy. Policies: Complete all clinical documentation following agency protocol and Medicare/Federal guidelines. Rules and Regulations: Understand and follow agency policies, procedures, rules, and regulations and communicate changes in schedule/availability to schedulers or supervisors. Operations: Attend in-service training and mandatory agency meetings.**HARD & SOFT SKILLS: Compassionate** communicator with a positive attitude. Patience is a virtue when working with patients, families, physicians, and coworkers. Attention to detail is critical, as is being observant and following directions.**REQUIREMENTS:**Speech Therapist with a current license in the state of employment.Minimum one-year experience as a ST in an acute care setting.
