Alternate Solutions Health Network in Lexington is seeking a Speech Therapist for home-based skilled care part-time (3 Days, 8:00AM - 5:00PM) covering the Lexington territory. You will collaborate with an interdisciplinary team to deliver compassionate therapy and educate families.

Qualifications

• Must hold a current state license as a Speech-Language Pathologist.
• Minimum of 1 year experience as ST in an acute care setting.
• Home care experience preferred.
• Valid driver's license and auto insurance in your name.
• Capable of performing all physical demands.

Responsibilities

• Develop and manage a Plan of Care for each patient.
• Guide patients in therapeutic, creative and self-care activities.
• Observe, document, and report patient progress to physician and agency staff.
• Improve swallowing, cognition, speech, and communication through home exercise programs.
• Minimize hospitalization risk through teaching and prevention.
• Counsel patients and families on disease, injury, and management across therapy phases.
• Complete clinical documentation per agency protocol and Medicare guidelines.
• Follow agency policies and schedule communications with schedulers.

HOW YOU'LL WORK:

You’ll provide home-based skilled speech therapy care for patients as directed by the attending physician. You will educate patients and their family members and ensures the safety of the patient.

MAJOR AREAS OF RESPONSIBILITY:

• Plan of Care: Develop an individualized ST plan of care and makes adjustments as needs change.


• Guide the patient in his/her use of therapeutic, creative and self-care activities for the purpose of improving functioning.


• Observe, record, and report patient reactions, changes, and progress to attending physician and any appropriate agency staff.


• Patient Outcomes: Use activities and home exercise programs to improve swallowing, breathing, cognition, speech and communication, and/or any other speech therapy related function.


• Minimize Patient Risk: Help decrease re-hospitalizations by teaching the patient on preventative measures and making good decision.


• Family Educator/Advocate: Counsel patient and family on the disease/injury and how to manage in all phases of speech therapy. Counsel and instruct patient and family on the disease/injury/disability and how to manage in all phases of speech therapy.


• Policies: Complete all clinical documentation following agency protocol and Medicare/Federal guidelines.


• Rules and Regulations: Understand and follow agency policies, procedures, rules, and regulations and communicate changes in schedule/availability to schedulers or supervisors.


• Operations: Attend in-service training and mandatory agency meetings.

• Attention to detail is critical, as is being observant and following directions.

REQUIREMENTS:

• Speech Therapist with a current license in the state of employment.


• Minimum one-year experience as a ST in an acute care setting.

• Valid driver's license and auto insurance in your name as a driver.


• Capable of all physical demands.
