Job summary

Alternate Solutions Health Network seeks a Speech Therapist to provide home-based skilled therapy in our Ohio service area. You will work with patients and families, delivering ST care under physician direction and educating families for continued progress.

The role requires an active SLP license and at least one year of ST experience in acute care or home health settings. Mileage is reimbursed, with a flexible schedule and a supportive team to assist with admissions and scheduling.

Qualifications

• Active SLP license in the state of employment.
• Minimum one-year experience as a Speech Therapist in acute care.
• Home health experience preferred.
• Valid driver’s license and auto insurance in your name.

Responsibilities

• Plan of Care: Develop an individualized ST plan of care and adjust as needed.
• Improve swallowing, breathing, cognition, speech, and related functions via home exercises.
• Educate patients and families on therapy goals and safety.
• Reduce re-hospitalizations through preventative guidance.
• Counsel patients and families on disease, injury, and management in all phases of therapy.
• Follow agency policies and Medicare/ Federal guidelines.

You will educate patients and their family members and ensures the safety of the patient.MAJOR AREAS OF RESPONSIBILITY:Plan of Care: Develop an individualized ST plan of care and make adjustments as needs change. Guide the patient in his/her use of therapeutic, creative and self-care activities for the purpose of improving functioning. Observe, record, and report patient reactions, changes, and progress to attending physicians and any appropriate agency staff.Patient Outcomes: Use activities and home exercise programs to improve swallowing, breathing, cognition, speech and communication, and/or any other speech therapy related function.Minimize Patient Risk: Help decrease re-hospitalizations by teaching the patient on preventative measures and making good decisions.Family Educator/Advocate: Counsel patient and family on the disease/injury and how to manage in all phases of speech therapy. Counsel and instruct patient and family on the disease/injury/disability and how to manage in all phases of speech therapy.Policies: Complete all clinical documentation following agency protocol and Medicare/Federal guidelines.Rules and Regulations: Understand and follow agency policies, procedures, rules, and regulations and communicate changes in schedule/availability to schedulers or supervisors.Operations: Attend in-service training and mandatory agency meetings.HARD & SOFT SKILLS:Compassionate communicator with a positive attitude.Patience is a virtue when working with patients, families, physicians, and coworkers.Attention to detail is critical, as is being observant and following directions.REQUIREMENTS:Speech Therapist with a current license in the state of employment.Minimum one-year experience as a ST in an acute care setting.
