Job summary

OhioHealth in Ohio seeks a Speech-Language Pathologist for Home Care in the Outpatient Rehab department. You will evaluate patients, develop and carry out plans of care, provide family instruction, and coordinate discharge planning with a multidisciplinary team to achieve optimal functional outcomes.

You will document progress, communicate with physicians, and supervise SLP students and CFY placements while contributing to department quality and safety initiatives. Day shift; 40 hours per week.

Qualifications

• Master's Degree in Speech Pathology.
• Licensed in the State of Ohio.
• Home Care requires valid driver's license and acceptable MVR.

Responsibilities

• Interpret patients' history and assess baseline function and deficits.
• Develop and implement a plan of care with treatment and reassessment.
• Provide discharge planning and patient/family instruction.
• Document in medical records and communicate with physicians and staff.
• Act as clinical instructor for students and CFY placements.
• Maintain patient productivity and participate in department functions.
