H2 Health is seeking a Speech-Language Pathologist to join our outpatient team in Bluefield, Virginia. You will evaluate and treat a diverse outpatient population, collaborating with a multidisciplinary team to maximize communication, cognition, swallowing, and independence.

Qualifications

• Master’s degree in Speech-Language Pathology from an accredited program.
• Current Virginia Speech-Language Pathologist license or eligibility for licensure.
• CCC-SLP preferred or eligibility for CFY.
• CPR certification or willingness to obtain as required.

Responsibilities

• Perform comprehensive speech-language evaluations and develop individualized plans of care.
• Assess and treat communication, language, cognitive-linguistic, voice, fluency, and swallowing disorders.
• Deliver evidence-based interventions to improve communication, cognition, safety and quality of life.
• Collaborate with patients, caregivers, physicians and interdisciplinary team members to achieve optimal outcomes.
• Educate patients and families on treatment strategies and home programs.
