Job summary

Evolve Therapy Services in Marion, OH invites a dedicated Speech Language Pathologist for an in-house Skilled Nursing Facility. The role focuses on planning, directing, and delivering SLP services to ensure high-quality resident care and compliance with federal and state standards.

The candidate will evaluate residents, create treatment plans with measurable goals, and train staff and families to apply therapy strategies in daily activities. Active licensure and CCC are required.

Qualifications

• Master's degree in Speech Language Pathology required.
• CCC is required for credentialing.
• Active SLP license in state(s) of practice.
• Ongoing continuing education to maintain licensure.

Responsibilities

• Provide quality speech therapy services per standards and productivity.
• Evaluate data for treatment planning and implementation.
• Develop and monitor treatment plans with goals.
• Instruct patients, families, and staff to carry over therapy.
• Complete CMS-compliant documentation in NetHealth EMR.
• Oversee FEES/MBS protocols and site program coordination.
• Participate in screening to identify resident needs with clinical excellence.

Job Position: Speech Language Pathologist for an In-house Skilled Nursing Facility

Job Type: Full Time

Job Purpose: To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions:

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.

• To evaluate and obtain data necessary for treatment planning and implementation

• To develop a treatment plan that includes a target list, short-term, and long-term goals

• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities

• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems

• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs

• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications:

• Master's degree in Speech Language Pathology

• Certificate of Clinical Competency

• MUST have active SLP license in state(s) of practice.

• Ongoing continuing education and professional development to maintain current licensure and certification
