Job summary

Evolve Therapy Services, LLC seeks a Speech Language Pathologist to provide comprehensive services at an in-house Skilled Nursing Facility in Minnesota.

The ideal candidate holds a Master’s in Speech Language Pathology, CCC, and an active state license, with strong assessment, treatment planning, documentation, and collaboration with residents, families, and staff; positions available full-time, part-time or PRN.

Qualifications

• Masters in Speech-Language Pathology required.
• CCC (Certificate of Clinical Competency) required.
• Active SLP license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide quality speech therapy services and initiate referrals as appropriate.
• Evaluate and obtain data for treatment planning and implementation.
• Develop treatment plans with target lists and short/long-term goals.
• Instruct patients, families and staff to carry over therapy goals.
• Complete CMS-compliant documentation in NetHealth EMR.
• Oversee FEES/MBS protocols and coordinate site programs.
• Screen residents to identify needs and ensure timely care.

Education

Master's degree in Speech Language Pathology

Certificate of Clinical Competency

Active SLP license in state(s) of practice

Continuing education to maintain licensure
