Job summary

Evolve Therapy Services, LLC in Minneapolis, MN, seeks a Speech Language Pathologist to provide in-house SNF services on a PRN basis, delivering skilled therapy and promoting resident well-being. This role requires a CCC and an active SLP license, with ongoing professional development.

Responsibilities include evaluation, treatment planning, and documentation in NetHealth EMR, plus oversight of FEES/MBS protocols and staff education for effective care delivery.

Qualifications

• Master's degree in Speech Language Pathology.
• Certificate of Clinical Competency.
• Must have active SLP license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Provide quality speech therapy services and initiate referrals when appropriate.
• Evaluate and obtain data necessary for treatment planning and implementation.
• Develop a treatment plan that includes target list, short-term, and long-term goals.
• Instructs patient, family, and staff in appropriate speech-language procedures for carryover.
• Complete CMS-compliant documentation in NetHealth EMR systems.
• Oversight and implementation of FEES/MBS protocols with site coordination.
• Participate in screening protocols to meet resident needs timely and with clinical excellence.
