Job summary

Evolve Therapy Services, LLC in Olivia, MN is seeking a Speech Language Pathologist for an in-house Skilled Nursing Facility. This full-time opportunity focuses on planning, delivering therapy, and thorough documentation in NetHealth EMR.

Required are a Master's in Speech Language Pathology, CCC-SLP, and an active state license with ongoing CEU participation. Relocation bonus and tuition reimbursement are offered; part-time or PRN options may be considered.

Qualifications

• Master's degree in Speech Language Pathology.
• CCC-SLP certification.
• Active SLP license in state(s) of practice.
• Ongoing continuing education to maintain licensure.

Responsibilities

• Provide quality speech therapy services in SNF setting.
• Evaluate and collect data for treatment planning.
• Develop and implement treatment plans with goals.
• Instruct patient, family, and staff on speech-language procedures.
• Document all therapy activities in NetHealth EMR per CMS standards.
• Oversee FEES/MBS protocols and site coordination.
• Screen residents to identify needs and ensure timely care.
