Job summary

Evolve Therapy Services in South Sioux City, NE is seeking a Speech Language Pathologist for an in-house Skilled Nursing Facility on a PRN basis. You will provide quality speech therapy services, develop treatment plans and educate residents and staff to ensure goals are carried over into daily activities.

The role requires a Master’s degree in Speech Language Pathology, CCC-SLP, and an active state license.

Qualifications

• Master's degree in Speech Language Pathology.
• Certificate of Clinical Competency.
• Active SLP license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Provide quality speech therapy services and initiate referrals as appropriate.
• Evaluate and obtain data for treatment planning and implementation.
• Develop treatment plans with targets, short-term and long-term goals.
• Instruct patients, families and staff to ensure carryover of therapy goals.
• Complete CMS-compliant documentation in NetHealth EMR.

Education

Master's degree in Speech Language Pathology

Certificate of Clinical Competency

Active SLP license in state(s) of practice

Ongoing continuing education for licensure
