Job summary

Highland Park Rehabilitation and Care Center seeks a Colorado-licensed Speech-Language Pathologist to join a large in-house therapy team. The role involves assessing and planning therapy for residents in long-term care, post-acute rehab, dementia care and outpatient clinics.

Qualifications

• Master’s degree in Speech Language Pathology.
• Colorado licensed Speech and Language Pathologist.
• Previous SNF experience preferred.
• Excellent communication skills, both verbal and written.
• Ability to work collaboratively within a multidisciplinary team.

Responsibilities

• Assess, plan, and implement therapeutic interventions to help patients reach maximum potential.
• Document patient progress and modify treatment plans as needed.
• Support patients and families with techniques to maximize function and independence.
• Develop care plans tailored to individual patient needs in a multidisciplinary setting.
