Job summary

Highland Park Rehabilitation and Care Center in Aurora, CO is seeking a Colorado-licensed Speech Language Pathologist to join our in-house therapy team. The position involves assessing and treating residents in long-term care, post-acute rehab, and dementia care within a 5-star facility.

Qualifications

• Master’s degree in Speech Language Pathology is required.
• Colorado licensed SLP is required.
• Experience in SNF environments is preferred but not essential.
• Strong written and verbal communication within a multidisciplinary team.

Responsibilities

• Assess, plan, and implement therapeutic interventions to maximize patient potential.
• Document patient progress and modify treatment plans based on ongoing assessments.
• Support patients and families with techniques to maximize function and independence.
• Develop care plans tailored to individual needs and maintain effective communication with IDT.
• Provide dysphagia, cognitive, and communication goals through evidence-based methods.
