Job summary

Banner Home Health in Mesa, AZ is seeking a licensed Speech Language Pathologist to provide autonomous, home-based rehabilitation for patients with speech, language, cognitive-communication, voice, swallowing, and fluency disorders. You will plan and deliver individualized therapy in the patient's home setting.

Qualifications

Masters degree in Speech Language Pathology from an accredited university program.
Current licensure in the state of practice. CCC or CFY eligibility. BLS certification.
Ability to work collaboratively with physicians and other healthcare providers.

Responsibilities

Evaluate patients’ functional abilities to determine intervention and goals.
Implement individualized treatment plans for speech, language, cognitive-communication, and swallowing disorders.
Educate patients and families; provide home programs with clear instructions.
Document treatment according to standards and regulatory requirements.
Participate in training to staff and students.
Follow plan of care and communicate with supervisor about unusual cases.
