Job summary

Family First Healthcare Services in Florida is seeking a licensed Speech-Language Pathologist (SLP) to provide high-quality home health speech therapy. You will assess communication, swallowing, and cognitive needs, develop individualized plans, and deliver evidence-based interventions in patients' homes.

The ideal candidate is patient-centered, organized, clinically strong, and committed to delivering exceptional home-based care, with Florida licensure and CCC-SLP preferred.

Qualifications

• Master’s degree in Speech-Language Pathology from an accredited program.
• Active Florida SLP license in good standing.
• ASHA CCC-SLP preferred.
• Excellent communication and patient engagement skills.
• Ability to work independently in home health setting.

Responsibilities

• Conduct comprehensive speech, language, cognitive, and swallowing evaluations in the home.
• Interpret patient histories, assessments, and physician orders to develop plans of care.
• Provide therapeutic interventions for speech, language, voice, cognition, and dysphagia as appropriate.
• Educate patients and caregivers on treatment strategies and carryover exercises.
• Modify treatment plans based on progress and evolving clinical needs.
• Collaborate with nurses, physicians, case managers, and the inter-disciplinary team.
• Maintain accurate, timely EMR documentation.
• Ensure adherence to federal, state, and agency standards.
• Stay current with evidence-based practices in speech-language pathology.
