Job summary

The ideal candidate is patient-centered, organized, clinically strong, and committed to delivering exceptional home-based care, with Florida licensure and CCC-SLP preferred.

Qualifications

• Master’s degree in Speech-Language Pathology from an accredited program.
• Active Florida SLP license in good standing.
• ASHA CCC-SLP preferred.
• Excellent communication and patient engagement skills.
• Ability to work independently in home health setting.

Responsibilities

• Conduct comprehensive speech, language, cognitive, and swallowing evaluations in the home.
• Interpret patient histories, assessments, and physician orders to develop plans of care.
• Provide therapeutic interventions for speech, language, voice, cognition, and dysphagia as appropriate.
• Educate patients and caregivers on treatment strategies and carryover exercises.
• Modify treatment plans based on progress and evolving clinical needs.
• Collaborate with nurses, physicians, case managers, and the inter-disciplinary team.
• Maintain accurate, timely EMR documentation.
• Ensure adherence to federal, state, and agency standards.
• Stay current with evidence-based practices in speech-language pathology.

Position Summary

You will assess communication, cognitive, and swallowing needs; develop individualized treatment plans; and deliver evidence-based interventions to help patients improve function, safety, and quality of life.

Key Responsibilities

• Conduct comprehensive speech, language, cognitive, and swallowing evaluations in the home

• Interpret patient histories, assessments, and physician orders to develop individualized plans of care

• Provide therapeutic interventions for speech, language, voice, cognition, and dysphagia as appropriate

• Educate patients and caregivers on treatment strategies, safety techniques, and carryover exercises

• Modify treatment plans based on patient progress and evolving clinical needs

• Collaborate closely with nurses, physicians, case managers, and interdisciplinary team members

• Maintain accurate, timely, and compliant documentation in the EMR

• Ensure adherence to federal, state, and agency regulatory standards

• Stay current with evidence-based practices and professional standards in speech-language pathology

Qualifications

• Master’s degree in Speech-Language Pathology from an accredited program

• Active Florida Speech-Language Pathologist license in good standing

• ASHA Certificate of Clinical Competence (CCC-SLP) preferred

• Strong communication, interpersonal, and patient engagement skills

• Ability to work independently and manage a home health caseload

• Compassionate, detail-oriented, and committed to high-quality patient care

• Proficient with computers and electronic medical record documentation

• Experience with Alora Plus EMR preferred

• (Preferred) Prior home health experience
