Qualifications

• Must have a Master's degree in Speech Language Pathology.
• Experience in home healthcare preferred.
• Ability to work independently in patients' homes.

Responsibilities

• Perform evaluations and provide therapy in patients' homes.
• Maintain accurate documentation of patient visits.
• Collaborate with interdisciplinary team for patient care.
• Identify, develop and participate in process improvement opportunities within the home health agency.
• Perform duties and responsibilities aligning with the service management philosophy toward patients, visitors, staff and other departments.
• Continually develop professional skills by engaging in ongoing continuing education.
• Advise and consult with family, interdisciplinary staff and physicians regarding communication and swallowing care.
• Maintain and complete documentation promptly and accurately including visit assessments, OASIS, 485 and Verbal Orders in electronic or written form.
• Provide speech, language, voice and swallowing evaluation and therapy for patients from infants to geriatrics.

Responsibilities:

• Perform duties and job responsibilities in a fashion which coincides with the service management philosophy of UPMC Home Healthcare towards patients, visitors, staff, peers, physicians and other departments within the organization.
• Continually develop professional skills by assuming responsibility for on‑going continuing education.
• Advise and consult with the family, interdisciplinary staff members, physicians and other professionals with regard to normal and difficult communication and swallowing and specific patient care.
• Maintain and complete all documentation promptly and accurately including visit assessment, OASIS, 485 and Verbal Orders on the laptop device or in written documentation while in the patient’s home and within the agency and regulatory standards. Data syncs (transmits) information same day.
• Patient care‑evaluation, therapy, teaching and counseling – provide complete and accurate speech, language, voice, swallowing evaluation therapy, instruction and counseling for infants through geriatrics.
