Personal Healthcare Management LLC in New York is seeking a Speech-Language Pathologist to assess and treat residents in a long-term care setting. The role requires a Master’s degree in Speech-Language Pathology, NY license, and ASHA certification, along with solid evaluation and treatment planning responsibilities.

You will screen residents, develop and implement care plans, document progress, and collaborate with the interdisciplinary team.

Qualifications

• Master’s degree required from an accredited program.
• Valid NYS license as Speech-Language Pathologist.
• ASHA certification in Speech-Language Pathology required.

Responsibilities

• Screen residents for rehabilitation therapy needs.
• Complete Speech-Language Pathology evaluations upon physician order.
• Establish Speech-Language Pathology plan of care per physician order.
• Provide Speech-Language Pathology treatments and modalities.
• Modify treatment program level, intensity and techniques based on resident response.
• Document progress toward goals and discharge when appropriate.
• Demonstrate Medicare Part A PPS knowledge and comply with regulations.
• Complete care plans and MDS as directed; assist in ordering necessary equipment.
• Participate in discharge planning with the interdisciplinary team.
• Maintain confidentiality and comply with facility policies; report suspected abuse.
