Job summary

Continuum Rehab Group is seeking a Speech Language Pathologist CF or CCC to join our in-house therapy team serving Skilled Nursing/Long Term Care Facilities in McGregor, TX. Qualifications

• Licensed SLP with CF or CCC eligibility.
• Experience evaluating and treating speech, language, swallowing, cognitive disorders.
• Ability to develop plans of care and document progress.
• Collaborate with OT/PT/physicians on patient treatment programs.

Responsibilities

• Evaluate and treat patients with speech, language, hearing, oral motor, swallowing, and cognitive disorders.
• Develop treatment plans with short-term and long-term goals.
• Document evaluations, progress notes, and discharge summaries promptly.
• Collaborate with therapists and physicians on patient treatment programs.
• Deliver quality rehab services in SNF.
