Job summary

Continuum Rehab Group is seeking a Speech Language Pathologist CF or CCC to join our in-house therapy team serving Skilled Nursing/Long Term Care Facilities in McGregor, TX. You will evaluate and treat patients to improve speech intelligibility and communication, using augmentative strategies as needed.

Responsibilities include treatment planning, documentation, and collaboration with a multidisciplinary team to deliver quality rehab services in the SNF setting.

Qualifications

• Licensed SLP in Texas (CF/CCC-SLP eligibility)
• Experience evaluating and treating speech, language, cognitive, and swallowing disorders.

Responsibilities

• Evaluate and treat patients to improve speech intelligibility and communication.
• Develop short- and long-term treatment plans and goals with physician orders.
• Document evaluations, progress notes, and plans of care per policy.
• Collaborate with therapists and physicians to coordinate patient care.
• Provide quality rehab services in SNF settings and ensure patient progress.
