Qualifications

• Licensed Speech-Language Pathologist in Indiana.
• Valid Indiana Operator's license and ability to travel for coverage.
• Basic computer literacy for documentation and communication.

Responsibilities

• Conduct comprehensive evaluations of residents' speech, language, cognitive-communication, and swallowing abilities.
• Coordinate admission/discharge to speech therapy with the Interdisciplinary Team and nursing staff.
• Provide therapy to residents under medical orders and in collaboration with the Care Plan Team.
• Monitor resident progress and adjust therapy techniques for optimal outcomes.
• Educate residents, families, and caregivers on communication strategies and exercises.
• Maintain accurate documentation of evaluations, treatment plans, and progress notes.
• Collaborate with physicians, occupational and physical therapists to coordinate care.
