Job summary

Continuum Therapy Partners is seeking a Speech-Language Pathologist to join our therapist-led team at Heritage Manor in Chisholm, MN, with a split-site arrangement at Hibbing, MN. Full- or part-time daytime schedules are available, offering comprehensive benefits and licensure support.

The role requires a Master’s in Speech-Language Pathology, an active state SLP license, and CCC-SLP certification, with preference for 1 year of experience.

Qualifications

• Master’s Degree in Speech-Language Pathology is required.
• Current, unencumbered state SLP License; ASHA CCC-SLP certification required.
• 1 year of experience as an SLP preferred.

Responsibilities

• Evaluates and develops individualized treatment plans for speech, language, and swallowing disorders.
• Documents patient progress and adjusts treatment plans based on outcomes.
• Educates patients and families on communication strategies and swallowing safety.

Speech-Language Pathologist Requirements & Responsibilities

• Master’s Degree in Speech-Language Pathology required
• Current, unencumbered state SLP License; ASHA Certificate of Clinical Competence (CCC-SLP) required
• 1 year of previous experience as a Speech-Language Pathologist preferred
