Job summary

Linwood Meadows Care Center in Visalia, CA is seeking a Full-Time Speech Language Pathologist to evaluate and treat residents with speech, language, cognitive-communication, and swallowing disorders. The role emphasizes timely assessments and collaborative care within a compassionate rehab team.

You will supervise CFY clinicians, develop treatment plans, document progress, and communicate with the interdisciplinary team.

Qualifications

• Bachelor’s degree in Speech Language Pathology required; Master’s or Doctorate preferred.
• Current California SLP license in good standing.
• Strong proficiency with electronic documentation (Casamba, PointClickCare preferred).
• Excellent communication, clinical reasoning, and organizational skills.
• Ability to supervise CFY clinicians and collaborate with restorative staff.

Responsibilities

• Evaluate patients within 24 hours or 1 business day of physician referral.
• Develop treatment plans and obtain physician approval.
• Administer treatments per the approved plan of care.
• Supervise SLP Assistants and CFY clinicians in patient care activities.
• Communicate effectively with the interdisciplinary team regarding patient status and progress.
• Attend and participate in Patient Care Conferences, UR meetings, and staff in-services.
• Document evaluations, daily notes, progress notes, recertifications, and discharge summaries per state and payer requirements.
• Provide caregiver and staff education for post-discharge maintenance programs.
• Ensure compliance with facility policies and the State Practice Act.
