Speech Language Pathologist - In-House SNF

Job Purpose: To assist in planning, organizing, developing, and directing Speech Language Pathologist services in accordance with current applicable federal, state, and local standards, guidelines, and regulations. The role includes following directives from DOR, Administrator, or Regional support team to ensure the highest quality of resident care is maintained.

Job Functions:

  1. Provide Speech Language Pathologist services to residents according to established standards, company procedures, therapy department protocols, and productivity standards.
  2. Adhere to company procedures regarding documentation and billing of services.
  3. Use sound judgment in evaluating, planning, implementing, and following up on resident therapy programs.
  4. Conduct regular screenings of residents to determine the need for intervention or treatment.
  5. Perform timely screenings of residents referred for physical therapy to assess intervention needs.
  6. Evaluate residents to gather data necessary for treatment planning and implementation.
  7. Conduct specialized evaluations as indicated.
  8. Develop treatment plans with short and long-term goals, including methods to achieve them.
  9. Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families, and interdisciplinary team members.
  10. Complete compliant and medically necessary documentation in accordance with CMS expectations using NetHealth EMR systems.

Education & Requirements:

  1. Minimum of a Bachelor of Science Degree in Speech Language Pathology from an accredited program.
  2. Active Speech Language Pathologist license in the state(s) of practice.
  3. Engage in ongoing continuing education and professional development to maintain licensure and certification.
