Responsibilities

Develop and implement individualized treatment plans tailored to each resident's needs and goals.
Conduct assessments, evaluations, and screenings to identify deficits and plan strategies for improvement.
Collaborate with the interdisciplinary team to develop and implement comprehensive care plans.
Communicate with residents, family members, and other healthcare professionals to ensure coordinated care.
Participate in staff development and orientation programs to maintain clinical expertise and professionalism.
