Job summary

Preferred Therapy Solutions is seeking a full-time Speech Language Pathologist (SLP) or CFY for a role split between East Hartford and Bloomfield, CT. You will provide comprehensive therapy in sub-acute/skilled nursing settings and collaborate with rehab staff to optimize patient outcomes.

Qualifications

• Proven ability to provide comprehensive speech-language pathology services.
• Current SLP license or eligibility to practice in CT.
• Ability to collaborate effectively with rehab/medical staff.

Responsibilities

• Deliver patient-centered SLP therapy in sub-acute/skilled nursing facilities.
• Collaborate with rehab therapists and nursing teams to optimize outcomes.
• Maintain timely documentation with strong computer skills.
• Be flexible with hours and scheduling in a two-site setting.
• Travel between East Hartford and Bloomfield, CT, with paid mileage.
• Provide ongoing patient assessment and discharge planning.
