Job summary

Alternate Solutions Health Network, LLC is seeking a Speech Therapist to provide home-based skilled therapy to patients in Darke County, Ohio. You will work with an interdisciplinary team to advance swallowing, speech and cognitive goals, educating families along the way.

The role offers a guaranteed base salary with an uncapped bonus structure designed to reward excellence. Schedule is Monday through Friday, 8:30 am to 5:00 pm, with no on-call or weekend duties."

Qualifications

  • Active Ohio license as Speech-Language Pathologist required.
  • One year of ST experience in acute care; home care preferred.

Responsibilities

  • Develop and implement individualized ST plans of care.
  • Educate patients and families on therapy goals and home programs.
  • Document patient progress per agency policies and Medicare guidelines.
  • Coordinate with attending physician and care team to optimize outcomes.

You will educate patients and their family members and ensures the safety of the patient.MAJOR AREAS OF RESPONSIBILITY:Plan of Care: Develop an individualized ST plan of care and makes adjustments as needs change. Guide the patient in his/her use of therapeutic, creative and self-care activities for the purpose of improving functioning. Observe, record, and report patient reactions, changes, and progress to attending physician and any appropriate agency staff.Patient Outcomes: Use activities and home exercise programs to improve swallowing, breathing, cognition, speech and communication, and/or any other speech therapy related function.Minimize Patient Risk: Help decrease re-hospitalizations by teaching the patient on preventative measures and making good decision.Family Educator/Advocate: Counsel patient and family on the disease/injury and how to manage in all phases of speech therapy. Counsel and instruct patient and family on the disease/injury/disability and how to manage in all phases of speech therapy.Policies: Complete all clinical documentation following agency protocol and Medicare/Federal guidelines.Rules and Regulations: Understand and follow agency policies, procedures, rules, and regulations and communicate changes in schedule/availability to schedulers or supervisors.Operations: Attend in-service training and mandatory agency meetings.HARD & SOFT SKILLS:Compassionate communicator with a positive attitude.Patience is a virtue when working with patients, families, physicians, and coworkers.Attention to detail is critical, as is being observant and following directions.REQUIREMENTS:Speech Therapist with a current license in the state of employment.Minimum one-year experience as a ST in an acute care setting.
