Job summary

Enhance Rehabilitation is seeking a qualified Speech Language Pathologist to join our team in providing patient-centered care in SNF settings. You will evaluate patients, develop and follow individualized plans of care, and monitor progress with the treatment team.

The role requires strong communication skills with patients, families, and staff, and experience with standardized assessments, swallowing evaluations, and treatment planning.

Qualifications

• Degree in Speech-Language Pathology (ASHA certification preferred).
• Experience in SNF settings is a plus.
• Excellent communication and collaboration abilities.

Responsibilities

• Provide patient care in SNF settings following individualized plans of care.
• Administer standardized speech/language assessments and swallowing evaluations.
• Develop and monitor treatment plans and progress with the treatment team.

• Experience with administering standardized speech/language assessments, conduct swallowing evaluations, developing treatment plans and monitoring patients' progress.
