Job summary

Blue Stone Therapy is seeking a Speech Language Pathologist (SLP) to provide therapy services to adult and geriatric patients at Accura Healthcare of Cascade in Cascade, IA.

The role focuses on evaluating and treating adult and geriatric patients in a skilled nursing and post-acute rehabilitation setting, including assessments, care plans, and treatment addressing dysphagia, cognitive-communication deficits, and speech impairments.

Qualifications

• Master's degree in Speech-Language Pathology from an accredited program.
• Current Speech-Language Pathologist license (or eligibility for licensure) in applicable state.
• Experience in skilled nursing, long-term care, or post-acute rehabilitation preferred.

Responsibilities

• Perform speech therapy evaluations and develop individualized plans of care.
• Provide skilled speech-language pathology services focused on functional and measurable outcomes.
• Treat swallowing disorders, dysphagia, cognitive deficits, communication disorders, and voice impairments.
• Participate in discharge planning and maintenance program development.
• Supervise Speech Language Pathology Assistants where applicable and per licensure standards.
• Maintain appropriate patient schedules per established guidelines.
• Complete timely, accurate documentation to support quality care and reimbursement requirements.
• Collaborate with interdisciplinary team members and facility staff to support positive patient outcomes.

Blue Stone Therapy is seeking a Speech Language Pathologist (SLP) to provide therapy services to adult and geriatric patients at Accura Healthcare of Cascade .

This role focuses on evaluating and treating adult and geriatric patients in a skilled nursing and post-acute rehabilitation setting.
