Job summary

Blue Stone Therapy is seeking a Speech Language Pathologist to provide therapy services to adult and geriatric patients in Inver Grove Heights and Minneapolis-area SNF/Transitional Care Unit settings.

Responsibilities include evaluations, care plan development, and treatment addressing dysphagia, cognitive-communication deficits, voice disorders, and speech impairments. Collaboration with interdisciplinary teams and accurate documentation are essential.

Qualifications

• Master’s degree in Speech Language Pathology from an accredited program.
• Current Speech Language Pathologist license (or eligibility for licensure) in applicable state.
• Experience in skilled nursing, long-term care, or post-acute rehabilitation preferred.

• Perform speech therapy evaluations and develop individualized plans of care.
• Provide skilled speech language pathology services focused on functional and measurable outcomes.
• Treat swallowing disorders, dysphagia, cognitive deficits, communication disorders, and voice impairments.
• Participate in discharge planning and development of maintenance programs.
• Supervise Speech Language Pathology Assistants where applicable and in accordance with licensure standards.
• Maintain appropriate patient schedules per established guidelines.
• Complete timely, accurate documentation to support quality care and reimbursement requirements.
• Collaborate with interdisciplinary team members and facility staff to support positive patient outcomes.

Speech Language Pathologist (SLP) - Inver Grove Heights / Minneapolis Areas

Blue Stone Therapy is seeking a Speech Language Pathologist (SLP) to provide therapy services to adult and geriatric patients at Ebenezer's Jones Harrison and Woodlyn Heights Senior Living in Inver Grove Heights (SNF/Transitional Care Unit setting).

This role focuses on evaluating and treating adult and geriatric patients in a skilled nursing and post-acute rehabilitation setting. The Speech Language Pathologist is responsible for assessments, care plan development, and implementation of treatment programs addressing dysphagia, swallowing disorders, cognitive-communication deficits, voice disorders, and speech and language impairments.
