Job summary

Blue Stone Therapy is seeking a Speech Language Pathologist (SLP) to provide therapy services to adult and geriatric patients at Cherokee Specialty Care in Cherokee, IA. The SLP will conduct assessments, develop treatment plans, and implement programs addressing dysphagia, cognitive-communication deficits, voice disorders, and speech impairments.

The role focuses on evaluations, plan development, and evidence-based care in a skilled nursing/post-acute rehabilitation setting, with collaboration

Qualifications

• Master’s degree in Speech Language Pathology from an accredited program.
• Current Speech Language Pathologist license (or eligibility for licensure) in applicable state.
• Experience in skilled nursing, long-term care, or post-acute rehabilitation preferred.
• Strong clinical reasoning, communication, and organizational skills.
• Commitment to patient-centered care and professional standards.

Responsibilities

• Perform speech therapy evaluations and develop individualized plans of care.
• Provide skilled speech language pathology services focused on functional and measurable outcomes.
• Treat swallowing disorders, dysphagia, cognitive deficits, communication disorders, and voice impairments.
• Participate in discharge planning and development of maintenance programs.
• Supervise Speech Language Pathology Assistants where applicable and in accordance with licensure standards.
• Maintain appropriate patient schedules per established guidelines.
• Complete timely, accurate documentation to support quality care and reimbursement requirements.
• Collaborate with interdisciplinary team members and facility staff to support positive patient outcomes.
