Olympia Transitional Care in Olympia, WA seeks a full-time Speech-Language Pathologist to join our in-house rehab program. You will screen and treat patients with communication, cognitive, or swallowing disorders, developing care plans and documenting progress within regulatory standards.

Responsibilities

• Screen and evaluate patients with communication, cognitive, or swallowing disorders, developing appropriate care plans.
• Deliver rehabilitative treatment to patients with communication, cognitive, and/or swallowing disorders, ensuring regulatory compliance.
• Evaluate treatment outcomes, modify services based on patient progress and coordinate referrals as needed.
• Provide consultation and counseling to patients, families, and caregivers related to speech disorders.
• Generate comprehensive discharge summaries complying with regulatory and clinical requirements.

Olympia Transitional Care in Olympia, WA, is a post-acute care facility with long-term care, short-term care, and geriatric services provided. We can offer a stable and fun in-house therapy team to work with as well as excellent programs serving our geriatric clientele in their journey to recovery. We are currently seeking a full time Speech-Language Pathologist for our in-house rehab program!

Key Responsibilities:

• Screen and evaluate patients with communication, cognitive, or swallowing disorders, developing appropriate care plans following regulatory and clinical standards.
• Deliver rehabilitative treatment to patients with communication, cognitive, and/or swallowing disorders, ensuring compliance with regulatory and clinical practice requirements.
• Evaluate treatment outcomes, modifying services based on patient evaluation and making referrals as needed.
• Provide consultation and counseling to patients, families, caregivers, and other service providers related to speech disorders.
• Generate comprehensive discharge summaries in adherence to regulatory and clinical requirements.

Qualifications:

Speech-Language Pathologist license is required.
