Job summary

Homeside Rehab is seeking compassionate and skilled Speech-Language Pathologists (SLPs) to join our growing home health team throughout Brooklyn, NY. You will provide individualized, evidence-based speech therapy services to patients in their homes while enjoying schedule flexibility and strong support from our clinical and administrative teams.

Current Brooklyn areas include East New York, Canarsie, Flatbush, East Flatbush, Crown Heights, Brownsville, Bushwick, Bedford-Stuyvesant, Brighton

Qualifications

• Active New York State Speech-Language Pathologist license required.
• Minimum 1 year of experience in home health, outpatient, or SNF settings preferred.
• Basic computer literacy for documentation and communication.
• Strong clinical and interpersonal communication skills.
• Ability to work independently in a home health environment.
• Reliable transportation for home visits preferred.

Responsibilities

• Evaluate patients and develop individualized treatment plans based on patient needs and goals.
• Provide evidence-based treatment for speech, language, fluency, articulation, voice, and related communication disorders.
• Maintain accurate and timely documentation of patient care.
• Collaborate with the clinical team to ensure coordinated and effective treatment.
• Communicate effectively with office staff, patients, caregivers, and families.
• Collaborate with other healthcare professionals to support comprehensive patient care.

Job Summary

Homeside Rehab is seeking compassionate and skilled Speech-Language Pathologists (SLPs) to join our growing home health team throughout Brooklyn, NY.

In this role, you will provide individualized, evidence-based speech therapy services to patients in their homes while enjoying schedule flexibility, consistent case opportunities, and support from our clinical and administrative teams.

Current Brooklyn Areas Include:

• East New York – 11207, 11208

• Canarsie – 11236

• Flatbush – 11226

• East Flatbush – 11203

• Crown Heights – 11213

• Brownsville – 11212

• Bushwick – 11221

• Bedford-Stuyvesant – 11216

• Brighton Beach / Sheepshead Bay – 11235

• Additional Brooklyn locations available based on patient needs

Requirements

• Active New York State Speech-Language Pathologist License – Required

• Schedule: Flexible scheduling with day and evening availability

• Monday through Friday
