Job summary

Homeside Rehab in Syracuse, NY is seeking a compassionate Speech-Language Pathologist to join our pediatric home health team. You will deliver individualized, evidence-based speech therapy services to patients in their homes, with schedule flexibility and a supportive team environment.

The role requires an active NYS SLP license, at least 1 year of relevant experience, and strong documentation and communication skills.

Qualifications

• Active NY State SLP license required.
• Minimum 1 year of experience in home health, outpatient, or SNF settings preferred.
• Strong communication and documentation skills.
• Ability to work independently in a home-health environment.

Responsibilities

• Evaluate patients and develop individualized treatment plans based on needs and goals
• Provide evidence-based treatment for speech, language, fluency, articulation, and voice disorders
• Maintain accurate and timely documentation of patient care
• Collaborate with the clinical team to ensure coordinated and effective treatment
• Communicate effectively with office staff, patients, and caregivers
• Collaborate with healthcare professionals to ensure comprehensive care

Job Summary

Homeside Rehab is seeking a compassionate and skilled Speech-Language Pathologist (SLP) to join our pediatric home health team in Syracuse, NY. Requirements

• Active NY State Speech-Language Pathologist License — required
• Minimum 1 year of experience in Home Health, Outpatient, or Skilled Nursing Facility (SNF) settings preferred
• Basic computer literacy for documentation and communication
• Strong clinical and interpersonal communication skills
• Ability to work independently in a home health environment
• Reliable transportation for home visits preferred
