Job summary

Emerald Hills Post Acute and Senior Living in Mount Horeb, WI, is seeking a full-time Speech Language Pathologist for its in-house rehab program. You will screen and evaluate patients, develop care plans, and deliver rehabilitative services tailored to geriatrics and wound care requirements.

Open to all experience levels, including new grads. Licensing is required; CAPLICO values guide daily practice and patient care philosophy.

Qualifications

• Valid Speech Language Pathologist license required (SLP).

Responsibilities

• Screen and evaluate patients with communication and/or swallowing disorders, and develop care plans aligned with regulatory standards.
• Provide rehabilitative, skilled, and medically necessary treatment interventions.
• Assist patients with functional activities; manage standing/walking, lifting, and transfers as needed.
• Administer procedures as part of rehab plans including modalities and wound care.
• Counsel patients, families, and caregivers regarding communication and swallowing disorders.
• Prepare comprehensive discharge summaries in compliance with regulatory requirements.

Emerald Hills Post Acute and Senior Living in Mount Horeb, WI, is a post-acute care facility with long-term care, short-term care, and geriatric services provided. We can offer a stable and fun in-house therapy team to work with as well as excellent programs serving our geriatric clientele in their journey to recovery. We are currently seeking a dedicated Full Time Speech Language Pathologist for our in-house rehab program!

Key Responsibilities

• Conduct effective screening and evaluation of patients with physical functioning disorders and/or wound care, developing appropriate care plans adhering to regulatory and clinical standards.
• Deliver rehabilitative, skilled, and medically necessary treatment interventions to patients with physical functioning disorders and/or wound care, in accordance with regulatory and clinical practice requirements.
• The role involves regular standing and walking throughout the facility and occasional lifting, transferring, and repositioning of patients and equipment. Physical demands vary throughout the day based on patient care needs.
• Administer various procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, use of supportive and assistive devices, and wound care.
• Provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders.
• Generate comprehensive discharge summaries of services provided, ensuring compliance with regulatory and clinical requirements.

Qualifications

• Speech Language Pathologist license is required.
• Open to SLPs at all experience levels. New Grads are welcome!
