Emerald Hills Post Acute and Senior Living in Mount Horeb, WI seeks a PRN Physical Therapist to join our in-house rehab program. The role involves evaluating patients with functional impairments, planning and delivering skilled therapy, and coordinating with the care team to support recovery and discharge planning.

Licensed PTs in Wisconsin are encouraged to apply; new grads welcomed. This is a PRN position offering flexibility to support our geriatric care services in a post-acute setting.

Emerald Hills Post Acute and Senior Living in Mount Horeb, WI is a post-acute care facility with long-term care, short-term care, and geriatric services provided. We can offer a stable and fun in-house therapy team to work with as well as excellent programs serving our geriatric clientele in their journey to recovery. We are currently seeking a dedicated PRN Physical Therapist for our in-house rehab program!

Key Responsibilities

• Conduct effective screening and evaluation of patients with physical functioning disorders and/or wound care, developing appropriate care plans adhering to regulatory and clinical standards.
• Deliver rehabilitative, skilled, and medically necessary treatment interventions to patients with physical functioning disorders and/or wound care, in accordance with regulatory and clinical practice requirements.
• The role involves regular standing and walking throughout the facility and occasional lifting, transferring, and repositioning of patients and equipment. Physical demands vary throughout the day based on patient care needs.
• Administer various procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, use of supportive and assistive devices, and wound care.
• Provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders.
• Generate comprehensive discharge summaries of services provided, ensuring compliance with regulatory and clinical requirements.

Qualifications

• Physical Therapist license is required.
• Open to PTs at all experience levels. New Grads are welcome!
