Job summary

Total Care Therapy is growing in Northeast Ohio and seeks a compassionate Speech-Language Pathologist to evaluate, diagnose, and treat communication, cognition, and swallowing disorders in assisted living residents. Qualifications

• Current state SLP license required.
• CCC-SLP certification preferred (or eligible).
• CPR/BLS certification required.
• Valid driver's license and reliable transportation.
• Experience in geriatrics, home health, assisted living, or SNF preferred.

Responsibilities

• Evaluate, diagnose, and treat communication, cognitive‑linguistic, and swallowing disorders.
• Develop individualized treatment plans to improve residents' quality of life.
• Perform dysphagia evaluations and swallowing interventions.
• Document progress in the EMR accurately and timely.
• Collaborate with physicians and care staff for coordinated care.

Setting: Assisted Living and Independent Living Communities

Available Locations:

Scheduling: Monday-Friday ⎸Part‑Time (Weekends available but not required)

Compensation: Per‑Visit (based on years of experience)

• Evaluations: $60-$70 Per Visit
• Reassessments: $45-$55 Per Visit
• Treatments: $45-$50 Per Visit
• Recertifications/ Discharges: $50-$60 Per Visit

About the Role

As a Speech‑Language Pathologist, you will evaluate, diagnose, and treat communication, cognitive‑linguistic, and swallowing disorders while developing individualized treatment plans that improve residents’ quality of life and functional independence.

Responsibilities

• Evaluate residents and develop individualized speech therapy plans of care.
• Assess and treat speech, language, voice, cognitive‑communication, and swallowing disorders.
• Perform dysphagia evaluations and implement appropriate swallowing interventions.
• Reassess patient progress and adjust treatment plans as needed.
• Complete timely and accurate documentation using the electronic medical record (EMR).
• Educate residents, caregivers, and facility staff on communication strategies, swallowing precautions, and home exercise programs.
• Collaborate with physicians, nursing staff, dietitians, and interdisciplinary team members to ensure coordinated, patient‑centered care.
• Maintain a productive clinical caseload while delivering compassionate, evidence‑based therapy services.

Qualifications

• Current State Speech‑Language Pathologist (SLP) License
• Current Certificate of Clinical Competence (CCC‑SLP) preferred (or eligibility if applicable)
• Current CPR/BLS Certification
• Valid driver’s license, reliable transportation, and auto insurance
• Experience in geriatrics, home health, assisted living, or skilled nursing preferred but not required
