Mason Creek Transitional Care in Katy, TX is seeking a full-time Speech-Language Pathologist for our in-house rehab program. The role involves screening, evaluating, and delivering rehabilitative care to geriatric patients, with a focus on regulatory compliance and patient-centered outcomes.

Responsibilities

• Screen and evaluate patients with physical functioning disorders and/or wound care; develop care plans adhering to standards.
• Provide rehabilitative and medically necessary treatment; follow regulatory and clinical requirements.
• Perform standing/walking, lifting and repositioning as needed during shifts.
• Administer rehabilitation procedures including manual techniques, ambulation, exercises, modalities, assistive devices, wound care.
• Provide counseling to patients, families, and other providers about physical disorders.
• Generate discharge summaries in compliance with requirements.

Mason Creek Transitional Care in Katy, TX, is a post-acute care facility with long-term care, short-term care, and geriatric services provided. We can offer a stable and fun in-house therapy team to work with as well as excellent programs serving our geriatric clientele in their journey to recovery. We are currently seeking a dedicated Full Time Speech-Language Pathologist for our in-house rehab program!

Key Responsibilities

• Conduct effective screening and evaluation of patients with physical functioning disorders and/or wound care, developing appropriate care plans adhering to regulatory and clinical standards.
• Deliver rehabilitative, skilled, and medically necessary treatment interventions to patients with physical functioning disorders and/or wound care, in accordance with regulatory and clinical practice requirements.
• The role involves regular standing and walking throughout the facility and occasional lifting, transferring, and repositioning of patients and equipment. Physical demands vary throughout the day based on patient care needs.
• Administer various procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, use of supportive and assistive devices, and wound care.
• Provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders.
• Generate comprehensive discharge summaries of services provided, ensuring compliance with regulatory and clinical requirements.

Qualifications

• Speech-Language Pathologist license is required.
• Open to SLPs at all experience levels.
