Job summary

Flagship Therapy in Jacksonville, FL is seeking a licensed Speech-Language Pathologist for a full-time in-house rehab program at Cedar Hill Post-Acute and Rehab. You will screen, evaluate, and treat patients with communication and swallowing disorders as part of our geriatric care team.

Qualifications

• Speech-Language Pathologist license is required.

Responsibilities

• Conduct screening and evaluation of patients with communication disorders and develop care plans.
• Deliver rehabilitative interventions for patients with communication disorders in a geriatric setting.
• Manage patient transfers, assistive devices, and wound care as part of rehabilitation plan.
• Provide counseling to patients and families and generate discharge summaries.
• Ensure treatments comply with regulatory and clinical standards.

Our expanding network of in-house therapy Ensign-affiliate operated healthcare facilities is growing, and we are scouting for excellent therapists to join us at Cedar Hill Post-Acute and Rehab in Jacksonville, FL. We can offer a stable and fun in-house therapy team to work with as well as excellent programs serving our geriatric clientele in their journey to recovery. We are currently seeking dedicated Full Time Speech-Language Pathologists for our in-house rehab program!

Key Responsibilities

• Conduct effective screening and evaluation of patients with physical functioning disorders and/or wound care, developing appropriate care plans adhering to regulatory and clinical standards.
• Deliver rehabilitative, skilled, and medically necessary treatment interventions to patients with physical functioning disorders and/or wound care, in accordance with regulatory and clinical practice requirements.
• The role involves regular standing and walking throughout the facility and occasional lifting, transferring, and repositioning of patients and equipment. Physical demands vary throughout the day based on patient care needs.
• Administer various procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, use of supportive and assistive devices, and wound care.
• Provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders.
• Generate comprehensive discharge summaries of services provided, ensuring compliance with regulatory and clinical requirements.
